We are excited to have you as our patient! On your first visit, pay the one-time enrollment fee and first monthly payment and receive the included benefits of the plan you choose. Most plans include a comprehensive exam, x-rays, and a professional cleaning. Patients diagnosed with periodontal disease will be required to be on our Perio Plan, and the initial scaling and root planing cleaning is billed out as a separate treatment procedure.

We have a Perio Plan specifically designed for you. It includes your regular perio maintenance visits as well as additional discounts on the initial scaling and root planing and other perio services. During your initial visit, we will let you know if these services are needed.

When paying your monthly subscription payments for our dental plan, Credit and Debit cards including MasterCard, Visa, American Express, and Discover are accepted. You can also use your HSA card. However, any payments made for treatment rendered will be in accordance with our office financial policy which states that cash or check payments are preferred at White Palm Dental. For patients on our dental plan who request to pay with a debit or credit card for any treatment rendered, a 3% transaction fee will be applied to payments made by card.

Your preventive regular care includes a doctor exam, annual x-rays and a professional cleaning. These are included at no additional charge and are available based on the visit frequency you choose when you sign up for your plan. Patients diagnosed with periodontal disease will be required to be on our Perio Plan, and the initial scaling and root planing cleaning is billed out as a separate treatment procedure. After the initial scaling and root planing cleaning is accomplished, all future perio maintenance appointments are included with our Perio Plan.

Our plans include a 20% discount on any other dental work that needs to be completed, (filling, crowns, implants, etc.) or on any elective work you may desire (veneers, cosmetic bonding, bleaching etc.). This saves you additional money when work is needed.

Just schedule the work you need and we will automatically apply the discounts. There is no paperwork or hassle to receive the valuable discounts. There are no limitations or waiting periods for current or future dental needs.

We plan on your membership continuing on a month-to-month basis for as long as you have teeth. The total monthly amount will be billed for a minimum of 6 months and is not cancellable prior. If you need to cancel after the initial 6 months give us a call and we will be happy to discontinue your plan.
